Student Immunization Records

All full-time students are required to submit immunizations records. Those enrolled in health science programs, including part-time students, have additional requirements.

                 *New Meningococcal Vaccine Requirement Fall 2018*

All newly enrolled full time students 21 years of age and younger must have received a dose of quadrivalent meningococcal vaccine (MenACWY) on or after 16th birthday.

Please return immunization records to the Student Immunization Records Office located in the lower level of the Nickerson Administration Building.

Health Science and Allied Health Students

All students enrolled in Allied Health programs including Nursing, Dental, Phlebotomy, Medical Assistant, Certified Nurse Aide, EMT, and Paramedic are required to submit a physician's certification that the student has received the required immunizations.
